Following a book published, this volume draws on the achievements of the second phase of "Providing Integrated Health and Social Care for Older Persons (PROCARE)" that consisted of empirical fieldwork and an analysis of model ways of working in the nine EU Member States. Its chapters have been written by "internationally mixed" teams.
